If you require your child to take prescribed medication in school, please complete the linked form (click 'form').

 Along with this, we will also ask you to sign a letter of consent (click the link) to acknowledge that you (the legal guardian) filled in the form and are asking for it to be administered in school.

Please do not send in any medication without making the school aware. Thank you for your cooperation.
